Job Description:
As a Senior Case Manager, the post holder will be an integral
part of the South West Provider Collaborative clinical commissioning function
providing clinical, operational and quality oversight in conjunction with the
Clinical and Programme Director and Head of Service. The post holder will
ensure safe and effective care pathways are in place for all service users in
specialised mental health, learning disability and autism care settings, within
NHS and independent sectors for the regions population.
Main duties of the job
The post
holder will (not exhaustive):
Provide clinical
leadership and senior pathway advice to support to quality of care and
experience for service users in Secure, CAMHS, AED & Perinatal; this
includes holding a small defined caseload.
Manage the group
of case managers within SWPC and any other staff deemed appropriate.
Provide clinical
support to case managers and local commissioners in relation to the defined
population, this will include projected mapping of future service use,
identification of users for future service provision, planning in terms of
future service needs.
Work closely with
other wider mental health (NHS & Local Authorities) and statutory bodies
across the region in terms of pathway management and interfaces as well as
supporting service developments and improvements.
Maintain a strong
quality & safety monitoring and oversight function for all of the services within SWPC
Support
the business in driving transformation as well as value for money in planning,
commissioning and service the role is designed to build a combination of
subject matter expertise and technical skills to develop a strong service delivery
Define current
processes, facilitate discussions and advise colleagues as to how best practice
might be adopted in the definition of future processes, document those
processes as they are agreed and oversee their delivery.

Qualifications
Essential
- Registered Mental Health / Learning Disability Nurse or other registered health or social care professional
- Educated to masters level or equivalent level of experience of working at a senior level in specialist areas.

- Hold current accreditation with relevant professional body.
- Must have recent experience in specialised mental health commissioning.
