Job Description:

We are seeking a Referral Coordinator to join our team as part of the exciting new Integrated Non-Custodial Services (INCS) contract, which has been awarded to DPT for the next seven years.

The INCS contract brings together CJL&D, Reconnect and the Mental Health Treatment Requirement (MHTRs) in order to deliver care and support for individuals to address their health care needs and offending behaviour from custody diversion through to release back to the community.

The Reconnect Service provides early intervention for vulnerable people as they prepare to be released from prison into the community. Reconnect work within a multidisciplinary arena to ensure high quality care for any health related matters for those integrating back into the community.

The MHTR service delivers MHTRs to individuals who have received a community sentence from the courts which has an MHTR as an element of the order. MHTRs are designed to support individuals with low to medium mental health needs where the court considers that an individuals mental health is linked to their offending.

The post-holder will work across both teams, supporting the manager and being the single point of contact for referrals into the services.

Main duties of the job

The post-holder will work across the two teams, supporting the manager and being the single point of contact for all referrals into the services. It will be the responsibility of the post holder to have an excellent oversight of all referrals, knowing what stage of triage they are at, where they are within the criminal justice system and will ensure all deadlines are adhered to. The Referrals Coordinator will also be responsible for maintaining excellent referral pathways into and out of the services and will work closely with partner agencies.

The post-holder will provide the full range of data collection and administrative support to the Reconnect and MHTR services, ensuring the services operate efficiently and effectively on a day-to-day basis.

Qualifications

Essential

  • Educated to degree level or equivalent in: Health/Social Care OR equivalent qualification OR experience
  • Registration with Professional Body if qualified healthcare professional

Knowledge

Essential

  • Knowledge and understanding of the legislative framework and direction of mental health, social care and housing, eg: Care Act, Mental Health Act, Mental Capacity Act.
  • Must have good understanding of the social care needs of people with mental health problems
  • Must have knowledge of and work in a recovery orientated approach with people who use mental health services
  • Knowledge of the Criminal Justice System

Desirable

  • An awareness of Health and Criminal Justice Organisations working with Adults & young people e.g. Probation, Youth Offending Services (YOS), Mental Health & Learning Disability Services, Drug & Alcohol services, Child & Adolescent Mental Health services (CAMHS)

Experience

Essential

  • Experience of care management and working with people with mental health difficulties
  • Experience of working with people with mental health difficulties in a community and inpatient/custodial setting
  • Experience of multi-disciplinary/multi-agency working, particularly regarding substance misuse, adult social care, probation, housing, accommodation and support providers
  • Experience of working with people who are hard to engage through their vulnerability, who are challenging in presentation OR employ risk behaviours including self-harm
  • Ability to assess and manage risk
  • Experience of using databases: inputting data and information accurately, retrieving data etc.

Desirable

  • Experience of administration and referral coordination
